Terms of Service Agreement

Any laboratory result data supplied to you is for the purpose of advising patients on health and medical issues. You and ACM Medical Laboratory agree that clinical records of patient and all information transmitted through ACM QuikReport shall be regarded as confidential and both parties shall comply with all applicable federal and state laws and regulations regarding the use and disposition of such records and data. You shall not provide patient data to any other person or entity, except as required by applicable law or to another healthcare professional for the purposes of consultation on health and medical issues related to that patient. The provisions of this paragraph shall survive the termination of this agreement.

Access to ACM QuikReport is limited to only you and other approved users within your office. You agree to notify ACM of any and all users to enable ACM to provide each user with their own password. You agree that neither you nor any other user within your office share your passwords with other individuals so as to allow unauthorized access to ACM QuikReport. You are responsible for any acts and omissions by your personnel's use of ACM QuikReport and for any damages incurred by you as a result thereof. You agree to notify ACM Medical Laboratory immediately if a user leaves your employ or otherwise loses their privileges as a user so that ACM may remove their user name and password from the system. You are responsible for reporting to ACM Medical Laboratory any possible or actual violation of data security that comes to your attention. ACM Medical Laboratory reserves that right to remove access to the system for any user found abusing the system.
